Role Overview: Nurse The Social Worker adheres to CVM’s policies and regulations, supporting the social shelter team to provide essential health and support services to beneficiaries. Key Duties and Responsibilities Health Evaluation and Follow-Up • Conduct initial medical assessments to evaluate the physical health of shelter residents and identify any urgent health concerns. • Provide daily health monitoring for beneficiaries. • Deliver first aid, manage chronic conditions, and administer medications as needed. Health Education and Coordination • Educate beneficiaries on hygiene practices, nutrition, disease prevention, and chronic illness management. • Identify potential mental health issues and coordinate with mental health professionals for comprehensive care. • Connect beneficiaries with healthcare services, including primary care, mental health support, and substance abuse treatment when necessary. • Health Education for Minors • Develop and deliver targeted training sessions for minors (even small groups) on reproductive health and prevention of infectious diseases, including HIV. • Use age-appropriate language and materials to ensure understanding and engagement. • Facilitate interactive discussions to address questions, reduce stigma, and empower minors with knowledge on how to protect their health. • Coordinate with mental health and social work staff to provide a supportive environment for learning and discussion. • This responsibility reinforces CVM's commitment to holistic care and education, equipping minors with essential information for their well-being. • Medical Appointments and Crisis Management • Accompany beneficiaries to follow-up medical appointments at health centers, clinics, and hospitals. • Work with shelter staff and healthcare providers to establish a supportive environment. • Respond to medical emergencies and provide guidance during health-related crises. • • Nutrition and Meal Planning • Ensure that meals are balanced and nutritious, coordinating with the social worker to address any special dietary needs. • Collaborate with the cook to develop and update a weekly meal plan, revised quarterly. • • Resource and Documentation Management • Maintain and manage the shelter’s medicine stock and first aid kit. • Ensure adherence to prescribed treatment regimens for beneficiaries under medical care. • Reporting and documentation • Provide a monthly report on the beneficiary’s clinical status • Attend weekly meetings with CVM-CMS focal person in order to develop rehabilitation plans •
